Menopause & HRT Monitoring
Blood Testing Through Menopause and HRT
Perimenopause and menopause involve major changes in reproductive hormones, but blood testing is not routinely required to diagnose menopause in every woman. In women aged 45 and over with typical menopausal symptoms and changes in their menstrual cycle, diagnosis is generally based on symptoms rather than repeatedly measuring hormone levels.
Blood tests can still be valuable where the situation is less clear, symptoms could have another cause, menopause occurs earlier than expected or selected monitoring is required as part of Hormone Replacement Therapy.
YT Medical offers targeted Menopause & HRT Blood Testing based on the individual rather than promoting routine hormone panels where they are unlikely to provide useful information.
Which Tests May Be Considered?
Depending on your age, symptoms and treatment, testing may include FSH, Oestradiol, Testosterone, Thyroid Function, Full Blood Count, Ferritin, Vitamin B12, Folate, Vitamin D, HbA1c, Lipid Profile, Liver Function and Kidney Function.
FSH may have a role where menopause is suspected between the ages of 40 and 45, or where premature ovarian insufficiency is being considered in somebody younger than 40. It is generally much less useful for routine diagnosis in women over 45 with typical symptoms.
Oestradiol can be measured, but routine serum oestradiol monitoring is not required for most women using standard HRT. Levels fluctuate and can be influenced by the HRT formulation, timing of the dose and laboratory method, so treatment is usually guided more by symptoms and overall clinical response.
HRT Monitoring
The monitoring required during HRT depends on the medication being used and the patient’s wider health. Routine review should consider symptom control, side effects, bleeding patterns and relevant health risks rather than relying entirely on a hormone number.
Where testosterone therapy is being considered for an appropriate menopausal indication, a baseline testosterone level and subsequent monitoring may be required to ensure levels remain within an appropriate female range.
Other blood tests can be useful where symptoms overlap with other conditions. For example, Thyroid Function, Full Blood Count, Ferritin, B12 and Folate may help investigate fatigue or other non-specific symptoms, while HbA1c and a Lipid Profile can provide information about metabolic and cardiovascular health.
When Testing Is Particularly Useful
Blood testing may be considered where symptoms begin unusually early, menstrual changes are atypical, existing health conditions complicate treatment or a patient is experiencing symptoms that are not responding as expected.
It is also important not to assume that every new symptom during midlife is caused by menopause. Anaemia, thyroid dysfunction, nutritional deficiency and other medical conditions can produce overlapping symptoms and may require separate investigation.
Unexpected or persistent abnormal vaginal bleeding — particularly postmenopausal bleeding — requires appropriate medical assessment rather than simply adjusting HRT on the basis of blood results.
Understanding Your Results
Hormonal blood results should be interpreted according to age, symptoms, menstrual status and any hormonal medication being used. YT Medical can provide targeted testing, but results suggesting a condition requiring ongoing HRT prescribing, gynaecological investigation or specialist management should be followed through the appropriate clinical pathway.
