A Careful Approach to the Under-Eye Area

The tear trough is the groove that runs between the lower eyelid and upper cheek. In some people it becomes more pronounced because of natural anatomy, genetics or changes in facial volume with age, creating shadowing that can contribute to a tired appearance even when the person is well rested.

Tear Trough Rejuvenation requires particularly careful assessment because not every under-eye concern is caused by volume loss. Puffiness, prominent eye bags, skin laxity, pigmentation and fluid retention can all create a similar appearance but may not improve with dermal filler. In some cases, adding filler can make an existing problem more noticeable, so treatment is recommended only where the anatomy is considered suitable.

Who May Be Suitable?

Tear trough filler may be considered for selected patients with genuine hollowing or loss of support between the lower eyelid and cheek. During consultation, we assess the under-eye area, cheek support, skin quality, facial structure and whether puffiness or lower-eyelid bags are present.

Patients with significant bags, loose skin or a tendency towards persistent under-eye swelling may be better suited to another approach. Where surgery or specialist ophthalmic assessment is more appropriate, this should be considered rather than attempting to correct every concern with filler.

What Does Treatment Involve?

Where filler is appropriate, a carefully selected hyaluronic-acid product is introduced into the under-eye or supporting areas according to the patient’s anatomy. In some patients, improving support through the cheek may be preferable to placing filler directly into the deepest part of the tear trough, while others may benefit from a very conservative combination of both.

The aim is to soften the transition between the lower eyelid and cheek rather than completely eliminate every natural contour. Under-eye treatment is usually approached cautiously because relatively small quantities of product can create a noticeable change in this delicate area.

Recovery and Results

Temporary swelling, bruising and tenderness can occur after treatment, and the under-eye area can sometimes remain swollen for longer than other facial regions. The result should therefore be allowed to settle before further treatment is considered.

Hyaluronic-acid filler is temporary, although under-eye filler can sometimes persist for a considerable period. Maintenance should be based on individual assessment rather than routinely adding more product, as repeated treatment without appropriate review can contribute to puffiness or an overfilled appearance.

Important Risks and Limitations

The tear trough is a technically demanding treatment area and filler is not without risk. Possible complications include persistent swelling, visible or palpable product, contour irregularity, discolouration, asymmetry, infection and migration. Rare vascular complications can be extremely serious and may threaten skin or vision.

For this reason, YT Medical takes a conservative approach to under-eye filler and will not recommend treatment where the anatomy or expected benefit does not justify the risk. An alternative regenerative, skin-quality or surgical approach may sometimes offer a more appropriate solution.
